Hitting shots off the toe

I have been playing golf for quite awhile.
I have developed a terrible habit of hitting my shots off the toe of the club (I mean all clubs).I've tried everything from standing closer, etc. Please give me some advice on how to fix this. I did have a fifteen handicap which has now creeped up 19.
